Admission Application & Forms

The following forms are required components of the admission application, along with the campus visits and admission testing and screening.

1. Complete the General Application online
Submission of the General Application triggers a request for a formal student visit and admission testing and screening.  It includes general biographical information and an opportunity for the parent or guardian to describe the student candidate's strengths, challenges, and passions.  The General Application initiates the admission file.

2. Applicant Statement (Grades 6 - 12 only)
The Applicant Statement invites the student candidate to describe themselves with honesty, creativity, and clarity.

3. Transcript Request / Student Records
The Transcript Request is completed by a school official and sent with an official record of students grades directly to The Miami Valley School.

4. English Teacher Recommendation Form (Grades 6 - 12 only)
The English Teacher Recommendation Form is completed the student student candidate's English teacher and sent directly to The Miami Valley School.

5. Math Teacher Recommendation Form (Grades 6 - 12 only)
The Math Teacher Recommendation Form is completed by the student candidate's current math teacher and sent directly to The Miami Valley School.

6. Recommendation Form (Grades K - 5 only)
The General Recommendation Form is completed by the student candidate's current classroom teacher and sent directly to The Miami Valley School.

7. Optional Additional Recommendation Form
The Optional Recommendation Form is completed by someone who knows the student candidate in a non-academic context and sent directly to The Miami Valley School.
