MVS Moola / Scrip Program

This *shopping cart* fundraiser not only benefits our school, but every family, teacher and member of the staff. Great Lakes Scrip is our scrip provider and we suggest you visit www.glscrip.com and www.shopwithscrip.com to familiarize yourself with their website.

Family Order Form - This form includes the most popular retailers in our area, however you are always welcome to order from any of the retailers on the Master Retailer List. On one side of the Family Order Form there is a sample showing how to fill out the form and room to make any additions to your order.

When you place your first order, it is absolutely necessary that you sign the green Enrollment Form and to fill out the Waiver on the reverse side. Please return it to school with your order and check in an envelope marked MVS Moola/Scrip. We cannot fulfill an order until we have these forms. We also must know how you want to receive your order, so be sure to indicate your choice on the Waiver form. If your child is in the lower school, teachers will send down your order with attendance. Upper school families/students and staff can drop their orders off in the business office in the upper school in the designated box.

Scrip - Blank Form - Use this form when you find items you would like to order from the SCRIP master retail list, but they are not listed on our MVS form. This order form would not replace our standard order form; use this form for additional items only.

NOTE: Orders are now due by 9:00 A.M., not 10:00 A.M.as in past years. Pickup will be from 2:45 - 3:30 pm on Friday of the same week.
